Personal Data Protection Authority (PDPA) Clarification Text

As Prof. Dr. Osman Kelahmetoğlu, we attach maximum importance to the security and privacy of your personal data in accordance with the Law on the Protection of Personal Data No. 6698 (“KVKK”). This Clarification Text has been prepared to inform you about the purposes for which your personal data is processed, the legal reasons for processing, and the methods used.
 

1. Data Controller

Within the scope of KVKK, the data controller of your personal data is Prof. Dr. Osman Kelahmetoğlu. You can access the contact information regarding the data controller via the Contact page.
 

2. Processed Personal Data

  • Identity Information: Name, surname, T.R. identity number
  • Contact Information: Phone number, e-mail address
  • Health Information: Necessary medical data within the scope of the health services provided
  • Appointment and Transaction Information: Records regarding examination and treatment processes
  • Web Usage Data: IP address, access dates and times, browser information
     

3. Purposes of Processing Personal Data

  • Receiving, planning, and managing appointment requests
  • Carrying out medical diagnosis, treatment, and health services
  • Fulfilling legal obligations arising from the legislation
  • Responding to communication requests and evaluating feedback
  • Ensuring the security of the website and improving the user experience
     

4. Transfer of Personal Data

Your personal data may be transferred to authorized public institutions and organizations, hospitals and laboratories collaborated with within the scope of health service delivery, and to lawyers and financial advisors limited to legal and financial obligations, in accordance with Articles 8 and 9 of the KVKK and by taking the necessary technical and administrative security measures.

The transfer of personal data abroad is only possible provided that the conditions stipulated in the KVKK are met and, where necessary, your explicit consent is obtained.
 

5. Method and Legal Reason for Collecting Personal Data

Your personal data is collected through appointment and contact forms filled out on our website, communications made via telephone and e-mail, and during examination and treatment processes.

The data in question is processed based on the legal grounds of being explicitly provided for in the laws, the establishment or performance of a contract, the fulfillment of a legal obligation, the establishment, exercise, or protection of a right as specified in Articles 5 and 6 of the KVKK, and on explicit consent where necessary.
 

6. Your Rights Under KVKK

As a data subject, pursuant to Article 11 of the KVKK, you have the right to:

  • Learn whether your personal data is being processed,
  • Request information if it has been processed,
  • Learn the purpose of processing and whether it is used in accordance with its purpose,
  • Know the third parties to whom it is transferred at home or abroad,
  • Request correction of incomplete or incorrectly processed data,
  • Request deletion or destruction within the framework of the conditions stipulated in the KVKK,
  • Request notification of these processes to third parties to whom the data has been transferred,
  • Object to the occurrence of a result against you by analyzing it exclusively through automated systems,
  • Request compensation in case of damage due to unlawful processing
     

7. Application Method

To exercise your rights within the scope of KVKK, you can apply to us in writing or via the Contact page. Your applications will be finalized as soon as possible and at the latest within 30 days, depending on the nature of the request.
 

8. Data Security

In order to ensure the security of your personal data, all necessary technical and administrative measures are taken against unauthorized access, loss, unlawful processing, and disclosure.
